Propositions pour une éducation thérapeutique en situation de pluripathologie

Introduction : Les programmes d’education therapeutique actuels sont majoritairement centres sur une seule pathologie alors que plus de la moitie des patients sont en situation de pluripathologie. Les patients presentant certaines pathologies differentes ont des besoins educatifs communs. Objectif : Le but de ce travail est de proposer une modelisation de la prise en charge educative en situation de pluripathologie qui repondrait aux besoins des patients. Description : Le bilan educatif partage doit permettre une prise en charge adaptee et avoir un impact sur le programme d’education therapeutique. Afin de choisir parmi les ressources disponibles celles qui repondent aux besoins educatifs du patient, ce bilan educatif pourrait etre realise en amont de l’inclusion dans le programme. Le patient serait ensuite invite a participer a un parcours educatif personnalise en fonction de ses besoins. Celui-ci pourrait comporter des seances des programmes ou prestations educatives existantes, et/ou des seances du programme pluripathologie visant les competences transversales, et/ou des seances individuelles visant des besoins qui ne sont pas pris en compte dans l’offre existante. Conclusion : Le programme ideal n’existant pas, c’est bien a partir des besoins du patient, en prenant en compte et en adaptant les ressources disponibles, que le parcours educatif doit se construire.
