Introduction and characteristics of helicopter emergency medical services

헬리콥터 응급의료체계는 영미권의 응급의료 선진국에서 helicopter emergency medical services (HEMS)로 불리며 항공의료서비스의 일부이다. 응급의료 영역에서의 항공의료 서비스는 주로 항공이송을 지칭한다. 항공이송이란 현장 또 는 의료기관 사이에서 환자를 옮기는 데에 항공기를 이용하 는 것을 말한다. 항공의료서비스라는 용어는 항공이송 과정 을 전문화된 의료서비스를 제공할 수 있는 인력이 탑승하여 담당하고, 실질적인 환자 진료와 환자 감시를 시행할 수 있 을 때에 사용할 수 있다. 현장에서 병원으로 환자를 후송하 기 위하여 항공기를 이용한 최초의 사례는 제1차 세계대전 때부터 시작되었으며 오늘날에는 민간 항공이송 기관과 소 방 또는 군 등의 공공조직에 의하여 회전익 항공기로 불리는 헬리콥터와 고정익 항공기로 불리는 제트비행기를 이송수단 으로 항공이송이 이루어진다. 항공의료서비스를 가장 앞서 도입한 미국의 경우 전체 이송환자의 3%를 항공이송이 담 당하고 있으며, 전국에 걸쳐 300여 개의 HEMS 운영기관, 100여 개의 운영기지, 1,400여 대의 항공기가 운행 중이라 는 보고가 있다(2017 Atlas and Database of Air Medical Services, ADAMS) [1]. 본격적으로 HEMS가 도입된 1970년대 이후, HEMS의 적 용, 이송담당 인력의 고도화, 헬리콥터 및 의료장비의 발달, 헬리콥터 응급의료체계의 개요
