A patient with huge ectopic calcification that was diminished after conversion to prolonged hemodialysis

症例は35歳,男性.1998年にGoodpasture症候群にて血液透析を導入されたが,ブラッドアクセストラブルのため2000年より腹膜透析に変更された.2003年12月より高P血症が原因でCa・P積が90(mg/dL)2を超え,右肩に手拳大,左臀部に小児頭大の異所性石灰化を認めるようになった.2004年10月より血液透析へ移行されたが異所性石灰化は改善しなかった.2005年2月24日に母をドナーとした生体腎移植を希望し,当院を初診.石灰化部はGaシンチで集積を認め,排膿よりメチシリン耐性黄色ブドウ球菌が検出されたことから感染を伴う石灰化と診断され,腎移植は見送られた.以後血液透析の継続,抗生剤治療を行ったが,異所性石灰化は増悪し続けたため,週22時間の長時間透析が施行された.長時間透析開始後,Ca・P積は是正され,異所性石灰化は改善し,2009年1月14日に腎移植が施行された.高度に進展した異所性石灰化は治療困難であるが,その治療として長時間透析は有用であることが示唆された.巨大な異所性石灰化が長時間透析により改善し腎移植を施行できた報告はこれまでになく,貴重な1例を経験したので,文献的考察を加えて報告する.
