Proximal metatarsal osteotomy is a procedure for correction of moderate to severe hallux valgus and metatarsus primus varus. A recent report includes a technique of proximal Chevron osteotomy with distal soft tissue release and lashing of the first and second metatarsals toqether." That report described two incisions rather than the three incisions recommended for other types of proximal metatarsal osteotomies and distal soft tissue procedures." In this report, the same procedure using a proximal Chevron osteotomy is performed through a single medial incision. This permits all of the correction provided by the multiple incision techniques, but with considerably less postoperative morbidity as well as improved cosmesis. We have not found an increase in or additional complications using this technique.
