The role of alveolar macrophages in immune response in inflammatory pulmonary diseases is of the utmost importance and one of the key regulators of alveolar macrophages function is pulmonary surfactant protein D (SP-D). Multifunctional structure of SP-D offers the possibility to act as bivalent factor controlling macrophages phenotype and to determine the duality of the immune response encuring the possibility of proinflammatory or anti-inflammatory reactions. SP-D level is changes in different pulmonary diseases, therefore the protein can be used not only as a marker of pulmonary injury but as agent influencing on pathogenetic branches of inflammatory reactions.
