[Hypoglycemia associated with hyperinsulinemia in a subject with type 2 diabetes and liver cirrhosis].

症例は43歳男性. 29歳で糖尿病を発症し, 36歳からインスリン治療. 今回, アルコール性肝硬変による出血傾向のために入院したが, 低血糖が頻発し, 著明な高インスリン血症を伴っていた (血糖40～50mg/dl, 血清immunoreactive insulin (IRI) 142.8μU/ml). この時にはインスリンがCペプチドをモル比で上回っており, 肝硬変による肝でのインスリンの利用・分解低下で著明な高インスリン血症となった稀な一例と考えられる.
