A surgical case of thymic cancer associated with multilocular thymic cysts

症例は37歳，女性．検診にて胸部X線写真上右肺門部に前縦隔腫瘍を疑う異常影を指摘された．胸部CTおよびMRI検査で，多房性胸腺嚢胞および腫瘍性病変の合併を疑い，胸腔鏡補助下胸腺・胸腺腫瘍摘出術を施行した．腫瘍の一部が中葉中枢側に浸潤していたため，中葉を合併切除した．術後病理診断は多房性胸腺嚢胞に合併した胸腺類基底細胞癌；basaloid carcinomaであった．術後放射線療法を追加し，現在術後2年が経過するも無再発生存中である．胸腺発生のbasaloid carcinomaは低悪性度群に分類されるものの非常に稀な疾患で，これまでに自験例を含め37例が報告されているのみである．診断，治療，生物学的特性に関する文献的考察を加えて報告する．
