Classification of dissecting aneurysm of the aorta and proposal of corrective operative method.

形態的変化の明らかな解離性大動脈瘤129例について,型分類を行い,各型に応じた根治的手術術式を考察した.型分類に際してはDeBakey分類に準拠したが,新たに弓部型,腹部型を加え,また亜型を設けた.1. I型は21例で62%にARを合併した.2. II型は10例で80%にARを合併した.三つの亜型に分類した.3. III型は80例で,III a型18例,III b型62例であった.腹部主要分枝を含まないものをすべてIII a型とし,さらに2亜型に分類した.8例に上行大動脈への逆行解離を認めた.4.弓部型は12例で,解離の範囲によって2亜型に分けた.5.腹部型は6例で,2亜型に分けた.6.型分類に応じて,選択すべき根治手術術式を考察した.
