PHOTODYNAMIC THERAPY IN DERMATOLOGY

Photodynamic therapy (PDT) is an investigational therapeutic modality utilizing light-activated drugs for the treatment of a wide range of malignancies, including skin cancer. Phase III clinical trials are in progress in approximately 40 locations in North America to evaluate the efficacy of PDT in treating tumors at various anatomic sites. Analysis of the data from these trials will presumably provide the FDA with justification for permitting general application of PDT for treatment and palliation of certain tumors at specified anatomic sites.' Generally, PDT involves the selective uptake of a nontoxic photosensitizer by malignant tissue with subsequent exposure to ligbt, initiating a photochemical process that ends in the preferential destruction of tumor cells. In this paper the background, technique, mechanism of action, and clinical application of PDT will be reviewed with emphasis on the use of PDT to treat dermatologic disease.
