A life-threatening situation occurred during routine administration of nitrous oxide analgesia. A patient was given 100% nitrous oxide in place of oxygen when a nitrous oxide tank was accidentally connected to the oxygen line. The pin index system was no longer functional. Anesthetic gas cylinders should be double-checked before use to avoid a catastrophe.
