Assessment of Oral Malodor in Dental Clinics

歯科外来における口臭測定の実際およびその考え方について述べる.口臭発生源の80%以上は歯科の専門領域である口腔由来であるといわれている.口臭の主要な原因物質は揮発性硫黄化合物(VSC)であり,歯周病の原因菌を含む嫌気性菌が産生する.口臭検査は官能検査による判定およびガスクロマトグラフやガスセンサーを使ったVSC濃度測定による機器分析が行われている.しかし,機器分析はあくまで補助的な検査方法で,最終的な診断には官能検査が重要である. 口臭強度は歯周病の重篤度をよく反映するため,歯周病の重要な臨床マーカーの一つであると同時に,VSCは細胞組織に対する毒性が強く,歯周病の増悪因子であるという病的意義も持つ.また,口臭は一般的な歯科治療患者にとっても関心が高く,患者に口臭への関心を向けさせることは,口腔衛生の向上に対する動機づけとして有効かつ意義がある.
