A case of metastatic gallbladder tumor of the hepatocellular carcinoma

症例は49歳男性．B型慢性肝炎にて他院通院中，高度脈管浸潤を伴う多発肝細胞癌を認めたため，インターフェロン併用動注化学療法を施行したところ，奏効し，外来通院していた．経過観察中に施行した腹部CTにて胆嚢内にポリープ状の腫瘤を認めたため，当科に入院した．各種画像検査の結果，胆嚢ポリープと診断したが，胆嚢癌の可能性も考えられたため，開腹下胆嚢摘出術を施行した．病理組織では腫瘍細胞が索状や胞巣形成性に増生し，胆嚢腫瘍基部のリンパ管および静脈に腫瘍の浸潤を認め，化学療法前の腫瘍生検組織と類似しており，肝細胞癌の胆嚢転移再発と診断した．肝細胞癌の胆嚢転移はこれまでにほとんど報告がなく，きわめてまれであると考えられ報告した．
