The Neuropathic Foot in Diabetes Part I: Blood Flow

The importance of neuropathy in the aetiology of foot lesions has been widely recognized,’ but their pathogenesis remains uncertain. Recent studies of the circulation in the neuropathic foot have shown that blood flow is increased due to arteriovenous shunting and that the peripheral arteries are dilated and tiff.^,^ These abnormalities may be responsible (in part) for the pathogenesis of the neuropathic complications of the diabetic foot4 and may also be important in acute painful neuropathy5. This review summarizes recent observations of the circulation in the neuropathic foot and assesses their role in the development of the characteristic features of the diabetic foot.
