Diagnosis and Treatment of the Carotid Artery in the Middle Ear

A n aberrant presence of the internal carotid artery (ICA) in the middle ear is a rare condition that results from injury or infection eroding the medial wall of the middle ear, a congenital anomaly, or an aneurysmal dilatation of this vessel in its petrous portion. Because this anatomic aberration is rare, it is frequently misdiagnosed, subjecting patients to surgical exploration for an erroneous diagnosis. There are many reports of intraoperative attempts to remove a suspected vascular tumor or middle ear effusion creating massive hemorrhage or cerebrovascular accidents as a result of misdiagnosing an aberrant carotid artery. '-lo Presented here is a safe and reliable method of intraoperative diagnosis based o n two encounters with dehiscent carotid arteries in the middle ear.
