A health questionnaire based on paper-and-pencil medium individualized and produced by computer. II. Testing and evaluation.

A computer-generated and computer-processed health questionnaire has been evaluated for patient acceptance and reliability of content. Various formats have been tested and it was concluded that a mark-sense paper-and-pencil questionnaire offered significant advantages over a cardsort questionnaire. Studies of administration techniques showed that administration by a specially trained clerk is advantageous.
