Funktionelle Hypoxie-Bildgebung mit FMISO PET zur Individualisierung der Strahlentherapie

Zusammenfassung Vorgestellt wird eine Strategie zur Adaption der Dosis an strahlenresistente hypoxische Tumoren (Dose Painting). Hierzu wurden 15 HNO-Patienten vor Beginn einer 70-Gy-Strahlentherapie mit 18F-FDG und mit dynamischem 18F-FMISO PET untersucht. Nach ca. 20 Gy wurde ein weiterer dyn. 18F-FMISO Scan durchgefuhrt. Die voxelbasierten 18F-FMISO-Daten wurden mit einem kinetischen Modell ausgewertet, das die Berechnung lokaler Gewebeparameter fur Hypoxie und Perfusion ermoglicht. Eine statistische Auswertung ergab, dass nur eine Kombination aus beiden Parametern eine signifikante Stratifizierung hinsichtlich des Therapieerfolgs zulasst. Eine Ubersetzung der Bildgebungsdaten in geeignete Dosisverschreibungen kann nur uber ein TCP-Modell, das die funktionellen Parameter berucksichtigt, erreicht werden. Ein solches Modell wurde mit den Nachsorgedaten der 15 Patienten kalibriert. Somit ist eine Berechnung von lokal variierenden Dosiseskalationsfaktoren moglich, die dann zur Planung eines Hypoxie Dose Paintings verwendet werden konnen. Eine Planungsstudie zeigte, dass Dose Painting ohne eine hohere Belastung der Risikoorgane moglich ist.
