Atypical Presentation of Posterior Reversible Encephalopathy Syndrome after Post-dural Puncture Headache

Posterior Reversibl Ensefalopati Sendromu (PRES), baş ağrısını takiben görme kaybı, nöbet ve şuur düzeyinde değişiklik bulguları ile ortaya çıkan, sıklıkla ciddi hipertansiyon zemininde gelişen, çoğu zaman geri dönüşlü, olan nadir bir akut nörolojik tablodur. Genellikle kranyal manyetik rezonans (MR) incelemede, parieto-oksipital bölgeyi simetrik tutan vazojenik ödem görülür (1-3). Etiyolojisinde en sık preeklampsieklampsi, hipertansif ensefalopati, immünsupresif sitotoksik ilaç kullanımı, kemik iliği transplantasyonu, organ transplantasyonu ve otoimmün hastalıklar yer alır (1, 4). Klinik ve radyolojik bulgular en kısa sürede tanınır ve etiyolojik neden tedavi edilirse, bulgular kısa sürede tamamen kaybolabilir. Ancak tanı ve tedavide gecikme, status epileptikus, yaygın iskemik infarkt, kanama gibi geri dönüşü olmayan hasarlara ve ölüme neden olabilir (5). Preeklampsi, PRES’in sık sebeplerindendir. Postpartum preeklampsi ise, gebelik hipertansiyonu olgularının %5,7’sini oluşturan nadir ve bu nedenle zor tanı konan bir durumdur (6). Spinal anestezi sonrası en çok görülen büyük komplikasyonlardan biri olan postspinal baş ağrısı (PSBA), özellikle obstetrik hastalarda sık görülür, çünkü kadın cinsiyet, genç yaş ve düşük vücut kitle indeksi gibi PSBA riskini arttıran faktörler söz konusudur (7-9).
