A Surgical Case of Mitral Regurgitation and Atrial Septal Defect with Myotonic Dystrophy

筋緊張性ジストロフィー症(myotonic dystrophy : MyD)は,稀な遺伝性筋変成疾患であり,手術適応の ある心疾患を合併することがある.その心疾患に対する外科的治療は周術期における呼吸筋麻痺による呼吸 障害や刺激伝導障害に伴う不整脈,開心手術中の低体温に伴う筋硬直などの様々な問題により,その外科的 治療の適応外と判断されることが多く,またその手術報告も非常に稀である.今回我々はMyDを合併した 心房中隔欠損症(atrial septal defect : ASD)・僧帽弁閉鎖不全症(mitral regurgitaion : MR)に対し,胸 骨部分逆 L字型切開によるMICS法(minimally invasive cardiac surgery)を用いることにより,術後の 呼吸機能の低下を最低限に維持できた症例を経験したので報告する.日心外会誌 38 巻 2 号:119-122 (2009)
