A case of carotid-cavernous sinus fistula as a complication of carotid endarterectomy is presented. This complication has been seen in 4 cases (including the current presentation) since introduction, in 1967, of the use of the Fogarty catheter for extraction of arterial emboli and thrombi. Reevaluation of use of the stiff Fogarty catheter for endarterectomy is suggested.
