XXIII Use of Antibiotics in Stapedectomy: Histopathological Results

Pathological material in humans subsequent to stapedectomy is sparse. Lindsay and House have shown histological sections through the oval window region after a footplate shattering technique." There was no evidence of inflammatory sequelae. Brunner reported histological sections showing a membrane over the oval window after the stapes had been removed." Because of the paucity of human postoperative material, animal experiments have been performed recently to study the pathological changes resulting from oval window surgery. Singleton has shown in the case of the cat that four out of seven animals exhibited healing of footplate fractures by means of osteogenesis." In one animal which showed a complete bony union of a comminuted footplate, a collection of chronic inflammatory cells was present on the tympanic side of the footplate. Osteogenesis is to be expected as a response to any fracture but the presence of inflammation may also account for osteogenesis after surgery in the oval window. Schuknecht has demonstrated that osteogenesis commonly originates from
