reviewing the children and adolescents studied by Bruck and MacGillivary, they note that seven of the eight unstable patients described were receiving greater than 1.0 U/kg/day, a dose which, they stated, "our study had led us to consider as de facto evidence for overtreatment." This clear statement has certainly coincided with practice in many diabetes clinics for children in England. We have indicated in our article that this dosage limit can be too rigidly applied during adolescence. It would appear that we are in agreement with Dr. Rosenbloom that a dosage of 1.0 U/kg/day is one that we must be prepared to exceed, albeit with caution.
