Mediastinal cysts treated by video-assisted thoracic surgery

1993～1998年に当院で胸腔鏡下ないし補助下手術により摘出した縦隔嚢胞の9症例 (10手術例) を検討した.男性5例, 女性4例, 年齢は43～62歳であった.疾患別では気管支原性嚢胞4例, 胸腺嚢胞2例, 嚢状リンパ管腫1例, 胸膜嚢胞1例, その他2例であった.嚢胞の位置は上縦隔2例, 前縦隔3例, 中縦隔3例, 後縦隔2例であった.手術は全例胸腔鏡下に行い, ボートの数は3ヵ所が6例, 4ヵ所が1例, 小開胸追加が2例, 1ヵ所のポート及び前胸部縦切開による第2肋軟骨切除を加えたものが1例であった.術中, 開胸に移行した症例は2例であった.手術時間は平均2時間27分, 術後入院期間は平均13.9日であった.全例術後合併症は認めなかった.胸腔鏡下手術は侵襲が少なく, 良性疾患で, ポート孔からの標本摘出が可能な縦隔嚢胞性疾患に適した手術法であると考えられる.癒着が高度な症例に関しては, 隣接臓器損傷を伴わずに完全摘出するには適宜小開胸の追加が必要である.
