Minimising the Cause of Human Error

Accidents or incidents due to human error do not ‘just happen’ and it is rare for the cause to be as simple as human incompetence or negligence. This paper commences with a practical consideration of why we need to move away from this stereotyped answer and culminates with the reassurance that a comprehensive analysis is a useful and possible pursuit.
