Epidémiologie de l'insuffisance rénale terminale en Ile-de-France : enquête coopérative prospective en 1998

Une enquete epidemiologique prospective a ete conduite en 1998 pour determiner l'incidence et la prevalence de l'insuffisance renale terminale (IRT) en Ile-de-France et les caracteristiques des patients. La totalite des services de nephrologie et des unites de dialyse desservant la population de l'Ile-de-France ont participe a l'enquete. Le nombre total de patients atteints d'IRT requerant la dialyse de suppleance a ete de 1155 (dont 86 retours de transplantation et 29 enfants) pour une population totale de 10,7 millions d'habitants, soit 108/10 6 /an, l'incidence des nouveaux patients etant de 100/10 6 /an. L'âge moyen des primo-dialyses adultes (≥18 ans) etait de 59,8 ± 16,8 ans, avec 21,6% de patients âges de plus de 75 ans. Les nephropathies vasculaires representaient 22,5% et les nephropathies diabetiques 20,6%. Au total 36,5% des patients ont ete adresses au nephrologue moins de six mois avant le stade d'IRT. La duree mediane d'hospitalisation chez ces patients a ete de vingt-huit jours, contre trois jours chez ceux confies au nephrologue depuis au moins six mois. La prevalence des patients dialyses en Ile-de-France a augmente de 417 a 433/10 6 entre le debut et la fin de 1998, soit un increment de 3,8%, assume grâce a une augmentation du nombre de patients traites hors centre par autodialyse et par dialyse peritoneale.
