S-H-3. Surgical Treatment of Cerebral Aneurysms and Arterio-venous Malformations

(1) Arterial aneurysm The number of cases and its locations were, 55 internal carotid-posterior communicating (IC-PC), 13 middle cerebral, 5 vertebrobasilar and 4 multiple aneurysms. Seventy-three cases out of a total of 95, were treated by surgery with a post-operative mortality of 8 cases or 11 %. Intracranial manoeuvres such as clipping, trapping and coating were performed on almost all aneurysms except in 19 cases of IC-PC neurysms where cervical , ligations were done. Post-operative follow-up results revealed that death due to hemorrhage occurred in three patients, one inflicted with multiple aneurysms and treated by cervical ligation only and in the remaining two, having aneurysms in the anterior cerebral artery and anterior communicating artery which underwent craniotomy with muscle wrapping. Follow,up results showed that not a single cage of IC-PC aneurysm treated by cervical ligation died of hemorrhage, denoting that in cases of IC-PC aneurysm where clipping or trapping is considered to be impossible, cervical ligation would be a technique of choice.
