Caregiving problems and feelings experienced by family caregivers of stroke survivors the first month after discharge

The purpose of this study was to identify the major problems and associated feelings experienced by family caregivers of stroke survivors during the first month after returning home. Safety, difficulty in managing activities of daily living, and cognitive, behavioral and emotional changes of stroke survivors (for example, mood swings, lack of motivation, forgetfulness and memory loss, depression and calling the caregiver often) were the three most common problems experienced by caregivers during the first month. Other problems were loss of caregiver independence, confinement, tiredness and inadequate time to do caregiving tasks as well as managing stroke survivor physical symptoms, for example, pain, not eating and skin problems. The first month of caregiving is very dynamic and distressful for caregivers of stroke survivors and telephone contacts appear to be beneficial in assisting caregivers to cope with the caregiving process. Zweck dieser Studie war es, die Hauptprobleme und damit verbundenen Gefühle der Versorgung und Pflege von Schlaganfall-Patienten übernehmenden Familienangehörigen im ersten Monat nach der Klinikentlassung zu ermitteln. Besorgnisse um die Sicherheit und Schwierigkeiten bei der Bewältigung des Alltags sowie kognitive, verhaltensmäßige und emotionale Veränderungen (z.B. Stimmungsschwankungen Motivationsverlust, Vergesslichkeit/Gedächtnisverlust, Depressionen, häufige Inanspruchnahme des Pflegenden etc.) bei Schlaganfall-Patienten waren die drei häufigsten Probleme, mit denen die Angehörigen im ersten Monat zu kämpfen hatten. Weitere Probleme waren der Verlust der Unabhängigkeit des Pflegenden bzw. deren Gefühl des Angebundenseins Erschöpfung, mangelnde Zeit für die Übernahme von Pflegeaufgaben sowie der Umgang mit körperlichen Symptomen der Schlaganfall-Patienten wie z.B. Schmerzen, Nahrungsverweigerung, Hautproblemen. Die ersten vier Wochen nach der Entlassung sind für Pflegende von Schlaganfall-Patienten äußerst anstrengend und belastend und telefonische Kontaktaufnahmen scheinen vorteilhaft zu sein, um sie darin zu unterstützen, in den Pflegeprozess hineinzuwachsen. El objetivo de este estudio fue identificar los principales problemas y los sentimientos asociados experimentados por los familiares que cuidan a sobrevivientes de un ictus en el mes siguiente a su regreso a casa. Los tres problemas más frecuentes resultaron ser los referidos a la seguridad y a la dificultad para realizar las actividades de la vida diaria, así como a los cambios cognitivos, conductuales y emocionales (por ejemplo, inestabilidad emocional, falta de motivación, olvido o pérdida de memoria, depresión, demandas continuas al cuidador, etc.) del sobreviviente del ictus. Otros problemas fueron los relativos a la pérdida de independencia o el confinamiento del propio cuidador, a su cansancio y a la insuficiencia del tiempo disponible para realizar sus tareas como tal y para ocuparse de los síntomas físicos del sobreviviente del ictus, como dolor, inapetencia, problemas cutáneos, etc. El primer mes de cuidados es muy dinámico y agobiante para el cuidador y los contactos telefónicos parecen ser beneficiosos para ayudarle a afrontar el proceso de cuidado. Cette étude avait pour but de cerner les principaux problèmes et sentiments associés ressentis par les soignants membres de la famille de survivants d'un accident vasculaire cérébral au cours du premier mois de leur retour à domicile. Les trois plus fréquents problèmes rencontrés par les soignants au cours du premier mois sont la sécurité, la difficulté à gérer les activités de la vie quotidienne et les changements cognitifs, comportementaux et émotionnels des survivants d'un AVC (ex. sautes d'humeur, absence de motivation, manque/perte de mémoire, dépression, appels fréquents du soignant etc.). Les autres problèmes sont la perte d'indépendance/le confinement du soignant, la fatigue, le manque de temps pour s'acquitter des soins ainsi que la prise en charge des symptômes physiques des survivants d'un AVC tels que douleur, inappétence, problèmes dermatologiques. Le premier mois des soins est très dynamique et pénible pour ceux qui s'occupent des survivants d'un AVC et il semble que les contacts téléphoniques soient utiles pour assister les soignants dans le processus de soin.
