Anatomically Guided Neuronavigation: First Experience with the SulcusEditor

Summary Our first clinical experience with the SulcusEditor (SE) in neurosurgical patients with tumors in eloquent regions of the brain are reported. The SE is a software tool for the interactive delineation of the cerebral sulcal pattern in 3 D FLASH MRI data sets. The method has been implemented on RI C workstations under UNIX using an X-Window interface ba ed on the OSF-MOTIF Toolkit. The anatomical definition of sulci is supported by a three-plane display with linked cursor move­ ments. Thus problems in the definition of sulci tangent to the section plane are eliminated. The outcome of the segmentation procedure may then be passed to customary rendering software for 3 D visualization. So far the MRI data of 24 patients with tumors in eloquent regions of the brain have been subjected to the labelling procedure. Digi­ tization of 10 -12 sulci was considered sufficient for neurosurgical planning. Logistic requirements - the results of the SE analysis had to be made a available to the neurosurgeons preoperatively within a 48-h period after the MR acquisition could be fulfllied in more than 70 % of cases. Labelling takes approximately 4 h, the bidirectio­ nal data transfer in a heterogeneous clinical net­ work 2 h. Future developments will comprise improvements of the user interface as well as the intraoperative availability of the results.
