A 64-year-old male who had undergone kidney transplantation from a cadaveric donor for vascular kidney disease due to arterial hypertension is described. Following implantation of the kidney in the right iliac fossa immediate diuresis was observed with progressive improvement in renal function up to achieving a serum creatinine value of 1.7 mg/dL. Eleven days after transplantation the patient presented with intense pain in the area of the kidney graft and a decrease in urine output. The hematocrit value remained constant (26.1%). Doppler ultrasound demonstrated perirenal fluid collection of 9 cm × 3 cm not described in previous studies with no data of renal thrombosis (Fig. 1).
