567e surgeons can harvest a vastus lateralis muscle-sparing free flap faster than an anterolateral thigh flap. However, even if the flap harvest is faster, this does not overwhelmingly benefit the patient. Jones et al. demonstrated that prolonged operative time was not a predictor of medical complications in microsurgical free tissue transfer procedures of the head and neck.8 Rather than using speed and primary closure as reasons for flap selection, flap selection should be based on patient-specific factors and on the longterm flap viability.
