O CCASIONALLY the neurosurgeon is conf ron ted wi th a s i tua t ion in which he wishes he could safely approach and leave the cavernous por t ion of the caro t id a r te ry . Th i s paper records a case of t r a u m a t ic ca ro t id -cavernous fistula which was carried t h rough m a n y stages, inc lud ing definit ive surgery, and which i l lumina ted cer ta in re la ted a n a t o m i c a l observat ions .
