Case Management for People with Disabilities

In this paper we elaborate on case management for persons with disabilities. It is a strategy to help persons with disabilities and those directly concerned to find out what are their support needs and to bring together a set of support forms that is adequately geared to these needs. The social network as well as professional care workers from regular or specialised services can be mobilised. Clients are helped to co-ordinate, to follow-up, to evaluate and if necessary to adjust the support that is being offered. The process of case management is completed in five phases: assessment, planning, linking, monitoring and evaluation. We intend to show that case management may be a strategy to put new principles into practice. It aims at empowerment, person-centred support, social networking, inclusion, better co-ordinated and flexible services. The whole process takes place in full partnership with those concerned. In addition to the process and the principles, some organisational questions are raised that may be decisive for the success of case management.
