Use of dermoscopy to identify nail plate cavities as a clinical diagnostic clue for onychomatricoma

been reported to vary according to age and location. In patients under 12 years of age, globular-type CMN prevails whereas in older individuals, predominantly reticular pattern is observed. Furthermore, globular type CMN is more frequent on the trunk, whereas it is less evident on the extremities. Presence of yellowish–light brown globules and lack of reticular pattern in our case may be due to these ageand site-specific factors, and skin phototype. Skin phototype is an another factor affecting the predominant dermoscopic features in CMN. Seidenari et al. showed that CMN in fair-skinned individuals is mainly light brown type and on dermoscopy mostly shows diffuse pigmentation either alone or with globules, dots, or network. Zalaudek et al. reported that individuals with a fair skin typically show light brown to orange nevi with mixed pattern, which is mostly characterized by central hypopigmentation, and light brown color. In our case, different from that in fair-skinned individuals, we observed homogeneous skin-colored background rather than diffuse light brown pigmentation, which may be due to the lack of pigment synthesis in OCA. Furthermore, as there is a diffuse lack of background pigment in our albino case, we did not observe central hypopigmentation. Vessels have also been reported among dermoscopic features of CMN in different studies (8–68%). Linear, hairpin-like, dotted, comma, target vessels are the types that have been reported in CMN previously. In none of the previous studies were vessels the most dominant feature in CMN. Yet, in our case, vessels were the most dominant dermoscopic feature, which may be due to the lack of any other feature such as pigmentation. Although yellowish–light brown globules in our case suggest that some degree of melanin production is present, other classical features of CMN such as perifollicular hypo/hyperpigmentation, halo globules, dots, target globules, and homogeneous diffuse pigmentation in the background were not observed. Maybe with time, it will be possible to observe at least some of those findings as the nevus develops and the patient produces more pigment.
