Modified Right Ventricular Obliteration Technique for Fontan Candidates with Non-functional Right Ventricle

Fontan循環においては重症三尖弁逆流による右房・右室の拡大や,pulmonary atresia and intact ventricular septum(PA/IVS)などにおける著明な右室圧の上昇は右心系の機能障害だけでなく二次的な左心機能 障害や肺低形成の一因となる.当科ではこのような無機能右室を内腔より縫縮(内腔縫縮術)することで血 行動態より遮断する手術の工夫を行ってきたのでその有効性について検討した.対象は当施設で無機能右室 に対し内腔縫縮術を施行した 5例(6手術).心胸郭比は 71±10%から 61±5%へ有意に減少(p=0.017)し 左室短縮率は 27±17%から 37±5%へ変化した(p=0.071).全例が Fontan循環に到達し術後経過も良好で ある.新生児期に内腔縫縮術を施行した 1例で経過中に右室の再拡大を認めたため Fontan手術時に再度内 腔縫縮を追加した.当科で施行した右室内腔縫縮術は無機能右室を有する機能的単心室において有効な手術 方法であると考えられた.日心外会誌 45巻 1号:26-31(2016) キーワード:右室縫縮;Fontan;無機能右室
