A literature review on cementation of implant prosthesis

임플란트 본체와 상부보철을 연결할 때 나사유지를 이용해 왔으나 자연치 보철과 유사하게 시멘트유지형 임플란트 보철 을 이용할 경우 제작이 편리하고 비용이 저렴하여 널리 이용 되고있다. 시멘트유지형 보철은 나사유지형 보철물에서 흔히 합병증 을유발하는부적합(misfit)이발생할가능성이낮은것이장점 으로 꼽히고 있다. 또한 나사유지형 보철은 교합면의 중심와 에나사구멍(screw hole)이있어이상적인교합을만들어주기어 렵고 임플란트 장축방향의 힘을 유도할 수 없다. 나사구멍은 심미적으로도 문제를 유발하며 충전재료인 복합레진이 금속 이나 도재에 비해 빨리 마모되는 것도 임상적인 문제를 유발 할수있다. 무엇보다도재의부분파절(chipping)은나사유지형 (38%)에서시멘트유지형(4.5%)에비해10배가량빈번하게발생 하고있다. 임플란트 보철물은 추가적인 유지관리가 빈번하게 필요하 다. 나사풀림이나 파절이 있어 다시 조이거나 교환해야 할 때, 수복물의수리나변형이필요한경우뿐아니라임플란트주위 염치료를위해외과적인처치나치주관리가필요한경우에는 시멘트유지형보철물은잘제거될지확신하기어렵다. 반대로 임시합착제로합착한경우유지력이약하여수복물이자주탈 락하는 것도 문제가 된다. 다수의 임플란트를 연결하여 보철 을 한 경우 유지력이 너무 우수하다면 보철물 제거가 어려워 술자나 환자에게 힘든 과정이 될 수밖에 없다. 무엇보다 시멘 트를이용하여보철물을합착한경우잔여시멘트에의해부작 https://doi.org/10.4047/jkap.2016.54.4.458 REVIEW ARTICLE
