Case of a Patient Whose Initial Cardiac Event Was Ventricular Fibrillation during Sleep

要旨 13歳,中学 2 年生の女児.早朝に意識消失,四肢硬直を起こしているのを家族に発見され,すぐに父親による心肺蘇生が開始された.心室細動が認められ,救急隊による除細動にて発見から約 7 分後に心拍再開し当院集中治療室に搬送された.入室後集中管理を開始し,神経学的後遺症を残すことなく回復,除細動器を埋め込んで社会復帰した.特記すべき家族歴,既往歴はなかったが,中学 1 年次の学校検診心電図を取り寄せたところ,QTc時間0.54秒であった.QT延長症候群が疑われたため遺伝子検査を施行した.遺伝子検査の結果,LQT2と判明した.本症例は家族による救命蘇生術,救急隊による自動体外式除細動器の使用,当院における入院後の救命蘇生という救命の連鎖が有効であり,後遺症なく社会復帰できた貴重な例である.小児においても生命予後が不良な症例においては,植込み型除細動器を積極的に取り入れる必要があると考えられた.
