A Case of Infective Endocarditis with Incarcerated Vegetation in Mitral Orifice

症例は69歳，女性．発熱と呼吸困難を主訴に来院し，僧帽弁閉鎖不全兼狭窄症に合併した感染性心内膜炎（IE）によるうっ血性心不全と診断され，心不全および感染コントロール目的で入院となった．しかし，治療開始後も心不全が増悪するため，入院後9日目に準緊急で僧帽弁置換術を行った．術中所見では，僧帽弁口に疣贅が嵌入していた．僧帽弁狭窄症に合併したIEでは，常に疣贅の僧帽弁口嵌入の可能性を念頭において，より厳重な心エコー等によるfollowが必要であると考えられた．
