The paper deals with the application and results of a case study made on a selected source of emergency water supply in a selected region of the Czech Republic. The hazard identification to the assessed water source is based on a developed methodology and a compiled general register of selected types of hazards under the current acceptation of historical data, general and natural conditions, including an assessment of an environmental burden in the vicinity of the source and its infiltration area. The developed hazards register is the essential prerequisite for the subsequent assessment of the risk level of the water source and its classification within the system of crisis planning. Key-Words: crisis state, emergency water supply, groundwater, hazard identification, hazards register, source of emergency water supply
