Functional MR Imaging in Assessment of Language Dominance in Epileptic Patients

Twenty patients with intractable partial epilepsy were selected for this study. All LH or ambidextrous patients (11 patients) were included. Two RH patients in whom speech functions were supposed not to be represented – or not only – in the left hemisphere and seven RH patients in whom clinical data did not provide sufficient information for language lateralization were also included in the study. The presurgical evaluation included neuropsychological testing, video EEG (and stereoelectroencephalography (SEEG) in 10 cases), routine and functional brain MRI, and Wada test. French was the primary language of all subjects. The population consisted of 9 RH and 11 LH patients (14 males and 6 females), aged from 9 to 48 years (mean 25.8 years). The epileptic focus was located in the temporal lobe in 14 patients and in extratemporal areas in 6 patients. The left hemisphere was involved in 11 cases, 2 patients presented with a large frontal or hemispheric lesion. Introduction
