Self-Management of Complex Chronic Patients: Needs and A Proposal

Patient self-management plays an important role in an effort to deal more effectively with chronic conditions helping shifting resources into the community. This paper addresses self-management of complex chronic patients. General needs resulting from the literature are described together with concrete needs from the CONNECARE project. Finally, a proposal based on μservices, currently adopted in clinical trials running in 3 sites (Lleida, Israel, and Groningen), is presented.
