Trastornos de la personalidad en pacientes con trastornos de la conducta alimentaria

Se diseno un estudio de seguimiento para analizar la relacion entre los trastornos de personalidad (TT.PP) y la evolucion de los trastornos del comportamiento alimentario (TCA) en 34 mujeres que requirieron tratamiento por las alteraciones de la conducta alimentaria. El tratamiento se realizo a lo largo de 4 anos y medio. Se encontro una prevalencia de TT.PP del 91% en la muestra clinica en la evaluacion inicial, frente al 36% en la de seguimiento y una reduccion significativa en las puntuaciones en el MCMI-II. La resolucion del TCA se relaciono significativamente con la resolucion de los TT.PP y estos mejoraron en mayor proporcion en el grupo con bulimia (61%) que en el grupo con anorexia (34%). Las pacientes que presentaban trastornos de personalidad esquizoide y por evitacion fueron las mas resistentes y menos adherentes al tratamiento. Se comenta la prevalencia de los TT.PP en la muestra clinica y su relacion con el curso del TCA desde un modelo centrado en la nocion de persona. Personality disorders in eating disorder patients. A follow-up study was designed to analyze the relation between personality disorders (PD) and the course of eating disorders (ED) in 34 patients who required treatment over 4 years and half. 91% of the clinical sample met the criteria for PD at the initial assessment and 36% at the end of treatment, with a significant reduction in MCMI-II scores at followup. The outcome of the ED was significantly related to the PD outcome. There was a higher rate of improvement of PD in the bulimic group (61%) than in anorexic group (34%). The patients who presented schizoid and avoidant personality disorders were the most resistant and they adhered less to treatment. The prevalence of PD in the clinical sample and its relation to the course of ED from a person-centered model is discussed.
