Mesure semi-automatique des différents lobes hépatiques. Application à la recherche d'une corrélation entre volumes des lobes du foie. Une étude sur 50 angioscanners de foies sains

But de l'etude. - La volumetrie des tissus sains et pathologiques du foie est une donnee importante a considerer en hepatologie. A l'aide d'un logiciel specifiquement concu, nous avons evalue les volumes des 3 lobes de foies sains et recherche une relation entre volume total et volume des lobes. Patients et methodes. - L'etude a porte sur 50 examens scanner de foies sains a partir desquels ont ete mesures le volume du foie total et celui des lobes droit, gauche et de Spiegel. Les mesures ont ete realisees a l'aide du logiciel Hepato qui permet de caracteriser quantitativement et automatiquement les images tomodensitometriques du foie. Nous avons effectue une analyse de regression lineaire (methode des moindres carres) et calcule le coefficient de determination pour etudier la dependance entre les volumes mesures. Resultats. - Le volume hepatique moyen obtenu etait de 1497 ± 309 cm3, celui du lobe droit 1231 cm3, celui du lobe gauche 226 cm3 et celui du lobe de Spiegel 39 cm3. L'analyse statistique des mesures montrait l'absence de correlation significative entre les volumes des divers lobes hepatiques. Conclusion. - L'estimation du volume d'un lobe n'a pas ete possible a partir de la seule donnee du volume hepatique total et n'a pu etre realisee qu'a partir d'images tridimensionnelles confirmant l'interet des logiciels dedies a la mesure des volumes. © 2003 Editions scientifiques et medicales Elsevier SAS.
