Human Reliability Analysis

This chapter introduces human reliability concepts and covers different types of human reliability methods such as THERP (Technique for Human Error Rate Prediction), OAT (Operator Action Tree), ASEP (Accident Sequence Evaluation Program), HEART (Human Error Assessment Reduction Technique), STAHR (Social Technical Analysis of Human Reliability), SPAR-H (Standardized Plant Analysis Risk Human Reliability), and Bayesian networks. These methods are applied to safety, operational, and maintenance examples. This chapter also discusses the advantages and disadvantages of implementing each human reliability analysis method and shows how each method influences decisions using maintenance case studies. Methods for aggregating expert opinions are also discussed.
