A study using bupivacaine for continuous peridural analgesia in patients undergoing surgery of the hip.

SUMMARY Continuous peridural analgesia using bupivacaine 0.5% with epinephrine 1:200,000 in 22 unselected patients undergoing surgery of the hip is described. That the dose of bupivacaine is related to the age of the patient is confirmed by the results of this study. The latency of onset of analgesia was 6.1 minutes, the mean duration of the initial injection was 475 minutes, while that for subsequent injections was 255 minutes. Optimal conditions for the surgery described were obtained using this method, while excellent postoperative analgesia was provided. A minor degree of tachyphylaxis was apparent during the postoperative period, although this was not substantiated by statistical analysis. Continuous peridural analgesia is recommended as the anaesthetic of choice for operations on the hip. ZUSAMMENFASSUNG Es wird die kontinuierliche Periduralanaesthesie mit 0,5 % Bupivacain und Epinephrin im Verhaltnis 1:200.000 an 22 nicht ausgewahlten Patienten, die einer Huftoperation unterzogen wurden, beschrieben. Dass die Dosis Bupivacain mit den Alter des Patienten in Beziehung steht, wird von den Ergebnissen dieser Untersuchung bestatigt. Die Zeit bis zum Einsetzen der Analgesie betrug 6,5 Minuten, die durchschnittliche Dauer der Anfangsinjektion lag bei 475 Minuten, wahrend jede nachfolgende Injektion 255 Minuten anhielt. Mit dieser Methode wurden optimale Bedingungen fur die beschriebene Operation geschaffen und auch eine ausgezeichnete postoperative Analgesie erzielt. Geringe Anzeichen von Tachyphylaxie traten postoperativ auf, obwohl dies durch statistische Analyse nicht erhartet werden konnte. Kontinuerliche Analgesie wird als Anaesthesiemethode der Wahl bei Huftoperationen empfohlen.
