Failure Mode and Effects Analysis for Super Dense Operations

The concept of operations for Super Dense Operations (SDO) has been established in the literature, and algorithmic approaches to SDO routing have been studied. To follow on with the development of SDO, this paper addresses a Failure Mode and Effects Analysis (FMEA) for SDO. A team of Subject Matter Experts (SMEs) was consulted in order to perform this FMEA; the main failure modes as well as their effects affecting the nominal SDO were identified and their potential impacts on the system were ranked. The results of this study will provide guidance for future developments of mitigation procedures to safely and efficiently overcome SDO off-nominal conditions.
