Ileocecal angiodysplasia in a patient with chronic renal failure

症例は糖尿病性腎症による慢性腎不全透析導入期の67歳男性. 血液透析導入を計画され入院したが, 特に誘因なく著しい下血をきたした. 下血以外の腹部症状は認めず, 上下部消化管内視鏡・赤血球スキャンでは出血源を同定できなかった. 一旦は自然に止血したが, その後も輸血を必要とする下血を繰り返した. 透析導入と同時に腹部血管造影を施行し, 回結腸動脈の流入動脈拡張, 流出静脈の早期描出および末梢での異常血管集簇を認め, angiodysplasia (AGD) が考えられた. 維持透析に移行し, 全身状態が安定した時点で回結腸動脈とその支配領域の回盲部腸管切除術を施行. 摘出部の血管造影や病理組織像より回盲部AGDの確定診断を得た. その後, 下血は完全に消失した.慢性腎不全の合併症として消化管出血は臨床上重要である. 特に下部消化管出血ではその原因同定に困難を要することが少なくないが, AGDも鑑別診断として常に念頭に置く必要がある. これまでの本邦での報告では, 最終診断には腹部血管造影を要した症例が多く, 治療については外科的切除, 経カテーテル的動脈塞栓術, 内視鏡的電気焼灼, ホルモン内服治療等が試みられてきた. 今後は患者背景や長期予後からみた各種治療法の適応について, 多数例での比較検討が必要である.
