RECONSTRUCTION AFTER PROXIMAL GASTRECTOMY: COMPARISON WITH SINGLE LOOP JEJUNAL INTERPOSITION AND JEJUNAL POUCH INTERPOSITION

上部胃癌に対する噴門側胃切除の再建術式を評価するため,当科で施行した空腸間置術と空腸嚢間置術を比較検討した. 1985年から2002年の間に噴門側胃切除を施行し空腸/空腸嚢を用いて再建した上部胃癌症例72例(空腸: 13例,空腸嚢: 59例)を対象とした.空腸間置術/空腸嚢間置術の手術時間,出血量,術後入院期間はそれぞれ229.5/181.6分, 157.3/124.4ml, 23.6/23.8日であり差はなかった.空腸間置術では92.3%に愁訴を認め,空腸嚢間置術の42.4%と比べて有意に多かった(P=0.0014).術後内視鏡所見では両群に差はなかったが,空腸嚢間置術群で観察が容易であった.術後体重・総蛋白値はいずれの時点でも空腸嚢間置術が良好であった.器械吻合による経済面での問題があるものの,空腸嚢間置術は優れた再建法であると思われた.
