How to perform EUS-guided hepaticogastrostomy easier and safer.

Endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) is a useful method for failed ERCP cases[1,2]. B2 route is technically easier than B3 route for puncture the bile duct, guidewire insertion, dilation the track and antegrade stenting. But, B2 route is commonly trans-esophagus with the conventional oblique-viewing EUS. Trans-esophagus puncture may cause mediastinitis and severe complications[3].
