Bright red nuclei

To the Editor: I read with interest the case of the bright red nuclei by Pittock et al.1 They present the MR images of a 64-year-old woman with subacute diplopia, dysarthria, ataxia, and mutism. Although the red nuclei are implicated, the lesions appear to be paramedian within the inferior thalamic nuclei. I wonder whether Wernicke-Korsakoff syndrome (WKS) was considered in the differential diagnosis. The biopsy suggested lymphoma, and the clinical worsening despite steroids would argue against the …
