The required data were collected during stages of systematic review and develop of researcher-made questionnaire. A total of 131 nurses were selected using Cochran's sample size formula. The collected data were analyzed by Analytic Hierarchy Process (AHP) using Expert Choice software. Results showed that the most important factors affecting the failure to report medical errors by nurses were, respectively, management-related factors (W = 0.595), nurse-related factors (W = 0.276), and factors related to the error reporting process (W = 0.128).
