2001 Westminster Publications, Inc., 708 Glen Cove Avenue, Glen Head, NY 11545, U.S.A. mal weight gain and growth. No urinary symptoms were elicited. He had occasional headaches, unrelated to his abdominal pain. There was no significant family history. Psychosocial issues, behavior problems, and adverse life events were not evident on direct questioning. Physical examination was unremarkable. Over the previous 6 months he had undergone 2 gastrointestinal assessments. The first included a complete blood count, erythrocyte sedimentation rate (ESR), liver enzymes, renal function studies, serum amylase, urine microscopy and culture, and upper gastrointestinal contrast study, all of which were within normal ranges. There was suspicion of distal esophagitis at endoscopy; however, histology of the esophagus produced normal findings. A 2-week trial of a proton pump inhibitor and an empiric lactose-free and subsequently dairy-free diet were of no benefit, and the pain was becoming more frequent and severe. A further gastrointestinal evaluation at a referral center included
